Our services include routine hospice, respite, continuous care, inpatient support, and integrative therapies such as massage, pet, and music therapy—offered throughout a 21-county service area.Position Summary:We are seeking a skilled and compassionate Admissions Nurse (RN) to serve as the face of our hospice team across communities in the Greater Twin Cities. This is a road warrior position perfect for nurses with prior hospice, oncology, or palliative care experience, either directly in hospice or in acute-care settings with patients nearing end-of-life. This role is integral to our growing team, with room for personal and professional fulfillment.Key Responsibilities:
  • Travel to patient residences and senior communities to complete hospice admissions.
  • Assess clinical eligibility and initiate services with warmth, clarity, and professionalism.
  • Educate patients and families on hospice philosophy, benefits, and next steps.
  • Collaborate with physicians, intake coordinators, and interdisciplinary teams.
  • Document all clinical findings and care coordination accurately and promptly.
Qualifications:
  • Active RN license in the State of Minnesota.
  • Experience in hospice, oncology, or palliative care required (home care or hospital).
  • Comfort discussing end-of-life care with patients and families.
  • Strong clinical assessment and communication skills.
  • Valid driver’s license and reliable transportation.
