Responsibilities:What You’ll Do:∙Administer medication in accordance with standards of practice.∙ Encourage social\rehabilitative skills and encourage self-care and independence.∙ Perform client treatments in accordance with Plan of Care∙ Assist the consumer in learning appropriate self-care techniques.∙ Prepare clinical and progress notes to document care provided.∙ Monitor, document accordingly and report any change in client condition to Clinical Director/Clinical Supervisor∙ Promote activities to achieve a safe and healthy environment.∙ Observe aseptic technique when performing client procedures/care.∙ Collaborate with other members of the clinical team to develop and improve the Plan of Care∙ Assist the client in achieving established goals in the Plan of Care developed by the RN.∙ Maintain productivity according to the standards of the agency.∙ Maintain license, CEUs and mandatory employment requirements.∙ Participate in client care meetings, in-services, orientation and staff meetings.∙ Provide service in accordance with agency’s philosophy, standards, policies and scope of practice ∙ Participates in Performance Improvement and adheres to HIPAA guidelines.∙ Other duties as assigned. - Valid State LPN/RN License
- Valid driver’s license and use of automobile is required due to the travel involved.
- Annual MUI (Incident) training as dictated by the State of Ohio Medicaid, Ohio Home Care Program and Department of Developmental Disabilities
- Must be able to read and understand various documents used by the agency including Plan of Care, flow charts, client charts, agency policies and procedures.
- Must be able to use a computer, laptop or tablet.
- Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
- Employees must promote excellent customer service and always provide exceptional client care, while staying within the rules of the agency and rules of each program served.
