At the time of an offer, determination of your offer will reflect your skills and experience as it relates to the position.***Requirements:Directs a high visibility Social Services function to support the residents, and their families, for the residents' overall wellbeing. Carries out all facility and regulatory requirements for Social Services in a timely and cooperative manner. Serves to foster community relationships and enhances public relations. Must demonstrate a high degree of positive facility support and work effectively with all staff levels to maintain mutual facility objectives.Essential Job Functions· Develop an individualized social service plan (with the Social Workers) for each resident to include referral, social history and assessment, family input, assessment of social/emotional needs, short- and long-range goals, discharge plan, case management (psychological/psychosocial support services, financial information/assistance, and community services) and provide assistance/assessment/referrals as needed.· Responsible to lead and manage the Social Services staff, including providing positive and constructive feedback, goal and expectation settings, and performance management.· Work collaboratively with nursing staff and other departments and disciplines to meet residents' psychosocial needs. Attend related meetings such as Risk, Behavior, IDC.· Review and renew social service plan as required.· Serve as discharge planning coordinator; develop a written interdisciplinary discharge plan for each resident within seven days of admission, coordinate periodic review and revision of discharge plan; coordinate community services as needed for implementation of discharge plan.· Assist residents on an individual basis to manage issues related to nursing home placement, family loss, chronic and terminal illness, behavioral problems, and other related needs/issues.· Conduct resident and family group meetings to provide an ongoing opportunity for emotional expression, education, and orientation and document visits and changes in status and needs.· Attend resident council meetings and other resident activities as requested by residents.· Communicate with families as needed to meet resident care needs.· Adhere to facility policies and procedures.· Present inservice education to staff on the psychosocial needs of the geriatric resident.· Completes and/or coordinates project assignments for the Administrator.· Involved with Resident Rights/ Resident Abuse Prevention and Reporting.· Coordinates in-house services to include dental, eye doctors, psych. Coverage, etc.· Point person for State of ME program health screenings.· Point person for all Guardianships, Rogers, Conservatorships, ensure all files up to date, assist with filing for new Guardianships, Rogers, Conservatorships, serve appropriate paperwork, assist with clinical team with completion on med certs.· Other duties as assigned.Education· Bachelor's Degree in Social work; Master's degree preferred.· Must be a graduate of a school accredited by the Council on Social Work Education.· Current licensure as a social worker in the state of Massachusetts, or eligible for licensure.· Must have two to five years' experience as a Social Worker in a nursing home participating in the Medicaid and Medicare programs.Skills & Abilities· Able to lead a team, including but not limited to; oversight of department performance, goal setting, effectively motivate and address performance, etc.· Relationship management skills to effectively partner with all staff levels, residents and family members under demanding emotional conditions and stress.· Coordination and oversight of social service planning to ensure residents are receiving proper care in a timely manner.· Strong communication skills, including but not limited to, conveying care plans, de-escalating situations, clear and concise messages, in a supportive tone. Also includes appropriate written documentation of resident and family visits.· Ability to manage deadlines and competing priorities.· Required to work flexible schedule to accommodate resident and family needs.Working Conditions & Physical Demands· Able to move, sit and/or stand, bend, stoop, squat, and or kneel to floor level with or without a reasonable accommodation.· Able to document (in a computer) resident assessments, notes, updates, etc., with or without a reasonable accommodation.· Able to communicate and understand the English language.Employment Requirements· A pre placement physical is required and then every two years thereafter.· Annual tuberculosis screening or proof of chest x-ray with history of positive reaction.· Necessary vaccinations, i.e., influenza, COVID 19, etc., as needed per State of ME and Covenant Health.Summary:Covenant Health Mission StatementWe are a Catholic health ministry, providing healing and care for the whole person, in service to all in our communities.Our Core Values:
