Perform other duties related to the provision of care and development of the community MIH system. The MIH Coordinator is responsible for day-to-day operations and demand of the Mobile Integrated Healthcare through collaborative efforts with all internal and external stakeholders on current MIH programs. Also supports existing health services; provides integrated health services in partnership with other health professionals; extends access to health services delivery in underserved and general populations, including primary care, public health, disease management, prevention and wellness, and mental health; and performs other duties as required. Participates in related projects.ESSENTIAL FUNCTIONS1 – AdministrativeCoordinates daily operations schedule of patients, paramedics and APC’s.2 - Leadership/SupervisionCoordinate staffing requirements and make adjustments in collaboration with field operations and management team. Communicate expectations, delegate work to appropriate team members and monitor performance of MIH team members. Meet daily, weekly and monthly with Management Team and MIH Team to discuss program and process updates and issues in order to seek resolutions.a. Prepare and review records and reports of activities performed by subordinate staff
b. Review MIH practices and procedures to assess their effectiveness and the need for improvements.
c. Attend meetings with subordinate staff and others to review and discuss operational needs, and enhancements to policies and procedures.
d. Schedule and change work assignments as needed.
e. Attend management training and operational programs applicable to MIH
f. Plan and develop efforts to enhance recruiting of new employees.3- Interdepartmental IntegrationProvide Program and team updates to all internal department stakeholders in order to ensure program goals, performance indicators and objectives are being met. Ensure interdepartmental needs are being met by collaborating with department operational supervisors. Participates in continuing professional development programs as appropriate. Participates in activities that promote the MIH, to include student recruiting, and program marketing events, as appropriate. Logistics liaison between MIH and Providers, Case Management and Ancillary Services Liaison between MIH and IT for epic uploads, faxes and forms Coordinates appointments and follow-up with Providers, Hospitals and Ancillary Services Liaison between MIH and outside vendor relations Assists with department pilots and data collection Develops and completes appropriate reports and templates Attends meetings as requested and available Reviews patient referrals from inside and outside the organization for appropriateness Manages work queue, placement requests and inbound call referrals Liaison with pharmacy to reconcile medication utilized in the field. Manages supply chain through Workday for daily use in the field Attends department trainings and meetings.JOB REQUIREMENTSMinimum Education - PreferredAssociate's degree or higher plus three years’ experience in Healthcare/EMS/MIH. Equivalent combination of education and experience considered.Minimum Work ExperienceThree years’ experience in Healthcare/MIH/EMS preferred.
Verification of an acceptable motor vehicle driving record, at time of hire, with annual verification required thereafter.Required additional Knowledge, and AbilitiesModerate word processing, database, presentation and spreadsheet skills required.
Basic email and internet skills required. Ability to learn specific job related software applications.
Must maintain a high degree of academic and practical knowledge in EMS, and must attend sufficient continuing education classes, courses and seminars both on and off duty to maintain annual EMS certification, as required by the State of MassachusettsLANGUAGE SKILLS
