- Lead the development of individualized support plans based on formal assessments, personal goals, and strengths.
- Facilitate person-centered meetings, monitor progress, and adjust plans as needed.
- Ensure services are delivered according to plan timelines and document all interactions accurately.
- Advocate for each person's dignity, independence, and self-determination.
- Link individuals to community-based services, resources, and supports.
- Support participation in Aspire's group events, workshops, and social activities.
- Serve as the main liaison for families, guardians, Aspire staff, and community partners.
- Maintain up-to-date documentation in each person's electronic record.
- Participate in team meetings, case reviews, and interdisciplinary collaboration.
- Support program outreach, recruitment, and continuous improvement initiatives.
- Participate in ongoing training and maintain certifications in CPR, First Aid, Abuse/Neglect, and Aggression Management (training provided).
- Passionate about person-centered care and committed to equity, dignity, and inclusion.
- An excellent communicator who can build trust and lead with empathy.
- Organized, proactive, and able to manage multiple priorities with attention to detail.
- Curious and open to learning, with a collaborative mindset.
- Associate's or Bachelor's degree in a related field.
- At least one year of experience supporting individuals with intellectual/developmental disabilities.
- Valid driver's license and good driving record.
- Ability to work flexible hours, including occasional evenings.
- Strong written and verbal communication skills.
- Proficiency in virtual meeting tools and Microsoft/Google applications.
- QIDP certification or eligibility.
- Experience in coaching, curriculum development, or group facilitation.
- Familiarity with support planning, goal tracking, and community-based services.
