Utilizes best practices and standards based on scientific principles, theoretical knowledge, and clinical expertise.Utilizes and promotes motivational and health-coaching techniques to engage the Care Team.Oversees Care Management team to assure reassessment and modification of the care plan as necessary to achieve health goals.Understand and apply federal and state regulations effectively. Demonstrate knowledge of ACO/ICM organizational policies.Supports program data collection and analysis for PAC CCNs systemwide.
