- Plan, support and/or accompany people to be a part of all aspects of activities in their homes and communities which includes upkeep of home; grocery shopping; banking; cooking; going to church, coffee shops, health clubs, etc.; support consumers in volunteering or jobs, seeing friends, going to dances, etc.
- Provide “Personal Care” -- this term refers to assistance with creating and maintaining a positive, personal appearance (clothing choices, dressing, bathing, grooming, etc.)
- Help with meals (planning, shopping, food preparation.).
- Assist with household needs and chores (laundry, cleaning, errands)
- Assist each person to develop and maintain friendships and family relationships. (This means different things for each different person. This may consist of something as small as purchasing a thank you card to helping someone reconnect with their family for the first time in years).
- Help people with hospitality in their homes and neighborhoods including being a host or hostess.
- Monitor and be aware of the nuances of someone's health and medications.
- Help people to effectively communicate their needs and wishes to others.
- Carry out plans that are the collaborative efforts of the team, from medical professionals, case managers or from guardians.
- Perform all aspects of job in a way that allows for consumers to have maximum choice, independence, and inclusion.
- Help consumers be seen in a positive light at all times diminishing negative behaviors and focus on abilities. Always represent self and RISE UP in a positive way.
- Set up rides for participants or cancel them, as necessary.
- Responsible for all grocery shopping, ensuring there are fresh food and beverages (as diets will allow), and at least 2 weeks' worth of supplies always. HC schedules the delivery/pick-up of the groceries, which requires excellent schedule management.
- Responsible for monitoring the paperwork in the home, such as; timesheets, MA sheets, Time Off Requests, log books, consumer information & folders, personal spending money, etc. There is to be always a stock of at least 2 weeks' worth of documents. HC will let the office know when there is less than 2 weeks' worth of documents.
- Responsible for monitoring medical supplies/equipment in the home: gloves, chucks, depends, setting up medications and medication sheets with RCM. HC will alert the office before items are running low so that there are always appropriate supplies and medications in the home.
- Responsible for reviewing the MAR(s) to ensure there are no errors. If there are errors noticed, the office will be notified right away.
- Serve as a liaison between the office manager and the house schedules
- Assist participants with all aspects of planning; appointments, birthdays, dates
- Assist participants host parties, or write letters to their family/friends
- Attend critical medical appointments with the RCM and take participants to non-critical appointments on their own, once they are an approved driver
- HC ensures the smooth running of the home at all times – even when working elsewhere, or through thorough pre-planning of home/consumer needs prior to having time off (both scheduled & unscheduled).
