Full-Time Tailored Care Management ExtenderWho Are We:InReach provides a wide variety of support and services across the lifespan for individuals with intellectual/developmental disabilities and their families. We focus on each individual's unique needs and deliver the highest quality services possible by ensuring that each individual receives the right support at the right time and frequency by caring qualified staff member in the most independent setting appropriate.What We Need:The primary responsibilities of the Tailored Care Manager Extender are to enhance the ability of the customer to lead a self-determined life by coordinating and facilitating opportunities and resources that promote formal and informal support, information, choice, and assist with building efficient care teams.Job Description:
  • Coordinates services/appointments, engages in health promotion activities and knowledge sharing
  • Provides/tracks referrals and provides information and assistance in obtaining and maintaining community-based resources and social support services
  • Participates in case conferences and supports Care Manager
  • Recognizes individual diversity and ensures individual is included in their support plan and is informed at the individual's level of comprehension
  • Maintains customer/family confidence, protects operations and customer health/safety
  • Maintains confidentiality
  • Develops plans that meet the individual's health and safety needs
  • Ensures receipt of assessments and is familiar with the types of PCP (PATH, MAP, and ELP)
  • Assists customer in coordinating and facilitating the plan, goal setting, and target outcomes
  • Completes a written Action Plan for the PCP and oversees the follow-up of PCP goals
  • Establishes relationships with integrated care team members and community resources
  • Provides education on services to all parties involved in the care and support of the individual
  • Maintains medical record compliance and quality
  • Performs duties with minimal supervision
  • Demonstrates knowledge of emergency procedures and assists in crisis situations
  • Driving and travel required
Qualifications and Licensing Requirements:
  • High School Diploma
  • Experience working with the I/DD population
  • Microsoft Office and Excel experience
  • Valid driver's license and safe driving record
  • Have access to reliable transportation 24 hours a day and be able to travel to the home of any customer at any time
Hours: Monday-Friday 9:00am-5:00pm; flexibility needed to meet the needs of customers/familyLocation: 4014 Monroe Rd, Charlotte, NC 28205, Oakhurst CommonsRate: $16-$16.50/hr based on experienceJob Type: Full-time; 40 hours per weekBenefits:
