Our goal is to ensure continuous improvement in all aspects of our health services, and your role will be pivotal in achieving this through hands-on patient care and outstanding case management.Duties and Responsibilities
  • Case manage patients on a day-to-day basis ensuring a seamless healthcare experience.
  • Oversee LVNs and home health aides to maintain high standards of care.
  • Conduct comprehensive in-home visits to assess patient health status and risk factors.
  • Perform OASIS assessments to initiate and revisit patients' care plans.
  • Coordinate care with physicians, specialists, and other healthcare professionals to maximize patient outcomes.
  • Implement patient care plans effectively, adjusting plans as patient needs evolve.
  • Provide education and support to patients and their families regarding healthcare management and preventive measures.
  • Maintain accurate and detailed reports and records of patient care, condition, progress, and problems addressed.
  • Adhere to all professional, ethical, and legal regulations as set by the healthcare industry and our facility.
  • Communicate consistently with team members regarding patient treatment plans and progress.
  • Participate in professional development opportunities to keep up-to-date with the latest in healthcare methods and technologies.
Requirements
  • Current Registered Nurse (RN) license required.
  • Minimum of 1 year of nursing experience required.
  • Proven experience in home health care or similar field preferred.
  • Demonstrated ability to manage case loads efficiently.
  • Exceptional skills in communication (both verbal and written).
  • Strong organizational abilities and a detail-oriented approach to case management.
  • Experience with OASIS-C1 or the latest version preferred.
  • Ability to travel to patients' homes within the assigned area.
  • Knowledge of clinical protocols and procedures in home health settings.
  • Empathetic and culturally sensitive to deal with patients from diverse backgrounds.
  • Physical ability to perform necessary tasks including, but not limited to, lifting and moving patients as required.
