Position SummaryThe primary purposes of the Licensed Practice Nurse (LPN) are to provide direct patient care, patient and family teaching, and work cooperatively within the interdisciplinary health care team in maintaining standards of nursing care under the direction of the Registered Nurse (RN) and within the framework of the Ohio Nurse Practice Act.Qualifications
  • Graduate of an accredited school of nursing.
  • Current, active licensure as a Licensed Practical Nurse through the Ohio Board of Nursing.
  • Maintain current, active BLS certification through the American Heart Association.
  • IV Certification preferred.

Responsibilities & Expectations
  • Demonstrates proficiency in basic nursing service, knowledge, and skills.
  • Demonstrates knowledge of and applies evidence-based practice.
  • Collects comprehensive data pertinent to the patient's health or the situation.
  • Identifies and reports changes in patients' status to Registered Nurse for evaluation.
  • Documents nursing activities accurately and in a timely manner.
  • Implements the identified plan, coordinates care delivery, and employs strategies to promote health and a safe environment under the direction of the Registered Nurse.
  • Administers treatments and other direct care; prepares and administers medications as prescribed; observes and evaluates patients' responses to medications and treatments; and identifies and promptly communicates adverse reactions to the Registered Nurse.
  • Collects specimens as ordered.
  • Interacts with and contributes to the professional development of peers, colleagues, and students.
  • Collaborates with patient, family, and others in the conduct of nursing practice.
  • LPN is a patient advocate in conjunction with the other members of the healthcare team and fosters patient safety, comfort, and satisfaction.
  • Attains knowledge and competency that reflects current nursing practice.
  • Adhere to professional standards and guidelines, including ethical actions, relevant statutes, rules, and regulations, during nursing practice.
  • Exhibits High Reliability Organization (HRO) tools and tones.
  • Responsible for timely review of emails, department communications and applicable learning assignments and completion as applicable.
Department Responsibilities & Expectations
  • Receive phone calls and messages and return calls to patients and/or family in a timely manner.
  • Triage patients and review with LP as necessary any acute & post op patient issues with LP and schedule acute visits as necessary.
  • Call patients post op to check on status and document.
  • Check LP's schedules frequently for any add-ons or updates.
  • Review and complete incoming and outgoing referrals.
  • Review the patient's medical record prior to visit to verify completeness, including having test results and breast images available.
  • Room patients in a timely fashion; verify patient identity and compare with chart. Obtain vital signs, height, weight, and current accurate list of medications and pertinent information needed for the LP to assess the patient.
  • Complete psychosocial distress screening on all new cancer patients.
  • Perform suture removal, staple removal, drain removal, and surgical site care.
  • Perform and assist with specimen collection with a valid LP order.
  • Assist with set up of office procedures, including both clean and sterile techniques.
  • Assist with office procedures including, but not limited to:
  • Punch biopsies
  • Colposcopies
  • Incision and drainage of abscesses
  • May scribe for providers as directed. All documentation will be reviewed and signed by the physician after making any necessary amendments.
  • Accurately arrange for various surgeries, procedures, and tests to be scheduled. This includes precertification, authorizations, physician orders, communicating with the patient, and adding to the LP's schedule if indicated.
  • Propose orders for approval by the LP.
  • Propose prescription refills for approval by LP.
  • Schedule urgent and non-urgent appointments.
  • Accurately complete medical forms and correctly complete and route billing information to appropriate staff. (Example: FMLA/disability forms etc.)
  • Copy and release medical records according to policy.
  • Document in the medical record accurately and timely.
  • Educate patient/family for ordered surgery, testing, and medications as indicated.
  • Educate patient/family on drain care and surgical site care.
  • Notify LP promptly of abnormal lab results, testing results, or critical results and review results and orders as necessary.
  • Notify patient of test results at the direction of the LP.
  • Restock and clean exam rooms after use.
  • Maintain confidentiality of information obtained while performing duties.
  • Medication prior authorizations
Working Conditions
  • Meal and break times are coordinated with other staff members to maintain adequate coverage.
  • Shifts are scheduled or changed due to the staffing needs of the unit/department based upon volume and acuity.
  • Work weekends and holidays as assigned or as applicable based on department needs.
  • Cross training depending upon department needs.
Department Working Conditions
  • Typically works 8 hours per day, Monday through Friday.
  • May require paid overtime before or after normal working hours.
  • Subject to frequent interruptions during performance of duties.
  • Expected to adhere to office policy and procedures.
  • Subject to blood and body fluid exposure.
