
CONSULTATIVE PHARMACIST

- Collaborate directly with nurses and hospice staff to develop effective, cost-conscious medication care plans.
- Educate healthcare professionals and caregivers on medication usage and adverse event prevention.
- Accurately document clinical interventions and patient outcomes in electronic health systems. Ensure all encounter documentation is complete, clear, and ready for transcription/processing.
- Maintain and update patient medication profiles and records in accordance with HIPAA and regulatory requirements.
- Consistently meet or exceed expected productivity benchmarks.
- Demonstrate exceptional communication and interpersonal skills in a remote or hybrid work environment.
- Maintain a professional, empathetic tone in all patient and team interactions.
- Engage with a multidisciplinary care team in a fast-paced call center environment.
- Support team success through shared learning, collaboration, and knowledge sharing.
- Participate in weekend rotations and contribute to team coverage as needed.
- Performs other related duties as assigned or apparent.
- This position does not have supervisory or managerial responsibilities.
- PharmD or B.S. in Pharmacy from an ACPE-accredited school of pharmacy
- Active pharmacy license in home state (Pennsylvania or Tennessee)
- Pharmacy licenses in other states or willingness to obtain additional licenses
- Experience in hospice, palliative care, or oncology settings preferred
- Previous experience working in a call center or remote clinical consultation environment preferred
- Familiarity with pharmacy benefit management (PBM) services preferred
- Demonstrates competency in equianalgesic dosing, generic and therapeutic substitution, formulary management and prior authorization procedures
- Demonstrates understanding, competency and adherence to all state and federal pharmacy practice rules and regulations
- Strong clinical assessment and decision-making skills
- Excellent written and verbal communication skills
- Proficient typing and computer skills are paramount to employee satisfaction and success while fulfilling responsibilities

- This position entails a high level of work performed at a computer terminal and telephone or headset equipment throughout the day
- Regular eye-hand coordination and manual dexterity is required to operate office equipment
