Home Health LPNThe Licensed Practical Nurse (LPN) will be responsible for providing professional nursing care within the parameters of the established care plan, under the supervision of a Registered Nurse and within the scope of practice of the Licensed Practical Nurse (LPN). The LPN performs routine clinical assessments, supporting the participant throughout the illness trajectory, and uses the nursing process with a holistic approach to provide individualized care to participants and their families in the PACE Clinic or home care setting. The LPN plays an integral part of the delivery care team and regularly participates with the PACE Interdisciplinary Team (IDT) in the provision of care to participants in the PACE program, while honoring participant and family goals and wishes, aiming to make the PACE experience meaningful and helpful.What is PACE?PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes.   • Current Colorado licensure as a Licensed Practical Nurse maintained throughout employment.
  • Graduated from an accredited school or nursing.
  • Minimum of one year LPN experience, hospital, LTC, home health.
  • Minimum of 1 year experience working with the frail or elderly, paid or volunteer.
  • Valid Colorado driver’s license, adequate automobile insurance and access to reliable transportation if working in the community setting
  • Excellent interpersonal and communication skills.
  • Demonstrated clinical expertise appropriate to the setting and scope of LPN practice.
  • Current CPR certification is maintained throughout employment.
  • Adequate computer literacy in MS Word, Excel, Access, Outlook
  • Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.
  • Effectively work with and build relationships with others.
What We Offer:
  • Competitive pay structure starting at $26.50-$29.32 per hour, depending on experience and education. $5 per hour differential pay for weekend shifts.
  • Schedule options - Friday-Sunday 8am-5pm OR Saturday and Sundays 8a-8p.
