- Provide inpatient social work services to patients in acute psychiatric care, with a focus on geriatric populations.
- Supervise Certified Nursing Assistants in the delivery of recreation therapy activities.
- Support treatment planning, care coordination, and interdisciplinary communication.
- Document assessments, interventions, and progress in medical records.
- Provide counseling and emotional support to patients and families during hospitalization.
- Develop discharge and aftercare plans to ensure continuity of care post-hospitalization.
- Document patient interactions and maintain compliance with hospital and program standards.
- Facilitate communication between patients, families, and the healthcare team.
- Connect patients and families with community resources such as housing, financial assistance, rehabilitation, and support groups.
- Collaborate closely with psychiatric and medical staff to ensure high-quality, integrated care.
- Master's degree in social work (MSW) required.
- At least 1 year of hospital experience is highly preferred.
- Prior inpatient psychiatric experience, especially with geriatric patients, is highly desirable.
- Basic Life Support (BLS) certification required within 90 days of hire.
- Nonviolent Crisis Intervention Training required within 90 days of hire.
