This includes coordinating resources, maintaining documentation, and ensuring adherence to all Pathways policies and procedures.
Educational Requirements:Bachelor’s Degree in Human Services or a related fieldMinimum of one (1) year of experience in Human Services following graduationEquivalent experience in a related field may be considered Essential Duties and Responsibilities:Provide case management services to assigned consumers based on individualized service plansCoordinate and link clients to appropriate community resources and servicesMonitor client needs and progress, updating service plans as appropriateMaintain timely and accurate documentation using electronic health record systemsParticipate in staff meetings, training sessions, and interdisciplinary team discussionsEnsure adherence to professional standards, ethics, and confidentiality requirementsPerform other duties as assigned Skills and Knowledge Required:Experience in a behavioral health setting is a plusCompetency in using computers and electronic devices, including electronic health recordsStrong organizational, communication, and time management skillsAbility to work independently and collaboratively with diverse populationsKnowledge of local community resources is beneficial Preferred Qualifications:Prior case management or social services experienceFamiliarity with behavioral health systems, services, and terminologyThis is a full time position
