
Advanced Practice Provider (NP/PA) – Hospitalist, Nocturnist

- Collaborates with interdisciplinary health care teams in diagnosis and treatment of conditions requiring medical intervention in an inpatient environment as a member of the Hospitalist services.
- Provides initial and ongoing assessment and management of urgent care issues at night related to internal medicine or critical care needs.
- Writes accurate and timely progress notes, procedure notes, and/or admission, discharge, and transfer orders.
- Orders, performs, and interprets screening and diagnostic procedures and tests, and appropriately communicates and documents information.
- Privileged to perform procedures beyond core as identified by the General Internal Medicine- Hospitalist services.
- Facilitates and communicates with patient, family, and staff to promote continuity of care across the continuum of care.
- Attend required training and meets key regulatory requirements, including those of the privileging institution, CMS, and the Join Commission.
- Evaluates quality outcomes for the patients receiving care of this team through participation in the development of quality indicators and outcomes dashboard.
- Acquires information on the most current development and trends in the Physician Assistant field and/or Nurse Practitioner field, through educational programs, in-services, professional associates, and current literature
- Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive, and psychosocial status.
- Orders and interprets laboratory and diagnostic tests.
- Develops pharmaceutical and non-pharmaceutical treatment plans.
- Monitors effectiveness of plan of care.
- Educates patients on disease and treatment plans.
- Performs procedures as trained and appropriately credentialed.
- Documents within the Electronic Medical Record (EMR) and completes other necessary documents.
- Collaborates and/or consults with physicians and other health care providers as appropriate.
- Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.
Nocturnist Rotating Schedule; Approx. 12-13 shifts/month; Sat-Sun 6PM-7AM or 5PM-6AMPosition Requirements:Preferred Experience: Any experience as an Advanced Practice Nurse or Physician Assistant preferred with particular emphasis on inpatient and hospitalist medicine experience.Minimum Education:Advanced Practice Nurse: Master’s Degree or higher in Nursing
