Day.We are seeking a Registered Nurse – Care Coordination Lung Navigator position with our Care Coordination Team.The ensures timely access and seamless coordination of services throughout the continuum of care for a designated patient population which includes cancer patients receiving chemotherapy concurrently or consecutively; newly diagnosed breast or lung cancer patients; patients requiring referral to a tertiary care provider; and patients with complex care or women’s health issues. The RN Care Coordination Oncology Navigator functions as an active health advisor, educator and advocate throughout the process of screening, diagnosis, and treatment of cancer or other conditions. This position may be responsible for facilitation of scheduling and coordination of follow-up care if necessary. This position shares knowledge with patients and families regarding internal and external support services and resources, and tracks data on patients to ensure completion of recommended treatment plan and follow-up care. The RN Care Coordination Oncology Navigator works directly with the health care team to deliver innovative health management practices to the patients and community of Aspirus.HOURS: Part-time; 0.7 FTE, 56 hours per pay period/28 hours per week.Experience/Qualifications
  • Bachelor of Science Degree in Nursing. Master’s degree in nursing or closely related field is desired.
Certification in Oncology required. * Requires previous experience working with medical and radiation oncology programs.
  • Possesses an active Michigan or Wisconsin RN nursing license.
  • Willingness to be a change agent and positive role model.

  • Possesses confidence to communicate and outreach to other community health care organizations and personnel.
