Summary:This position provides specialty clinical pharmacy services with evidence-based care for patients that range in age from birth to 100+ years. Services include disease management, comprehensive medication review and management and all clinical needs for both patients and the healthcare team. The goal of the specialty clinical pharmacist is to optimize the care of outpatients who are receiving complex specialty medications and achieve therapeutic outcomes in specific chronic disease states as specified by the quality management committee, governing body, and accreditation standards Responsibilities:(50%) Review and interprets clinician's patient care notes/assessments accurately for supportive clinical data needed for prior authorization approvals and development of care plan recommendations. Completes standard work while in patient visits and during telephone calls. Documents all clinical activities and interventions accurately and completely. Develop and refine disease and drug specific clinical protocols to maximize patient adherence and promote evidence-based cost- effective care. Provide clinical consultation and clarification to patients, practitioners, and other healthcare providers. Collaborate routinely with practitioners regarding medication therapy changes and recommendations per current standards. Suggest appropriate, cost- effective therapeutic alternatives to medical staff as needed. Coordinate insurance issues, hospital follow-up visits, medication reconciliation, refills, referrals, prior authorizations, financial assistance, and any other specialty related task. Monitor on-going patient adherence, program metrics and performance across all sites and specialty disease states.(25%) Evaluation of EMR to review: drug/ disease and drug/drug interactions, appropriate medication selection including dosage, labs/vitals, allergies, RX orders/renewals and direct physician communication, and other relevant information important to maximize medication adherence and optimize patient outcomes. Recommend alternative therapeutic drug regimens and disease specific treatments according to guidelines as appropriate. Monitor and evaluate therapies for side effects or other medication related issues, recommend drug therapy changes to prescribers as appropriate. Medication therapy management (MTM): including reviewing and monitoring of medication regimens, advising clinicians on evidence-based therapy, documenting interventions; and assessing patient adherence rates. Clinical interventions including but are not limited to comprehensive pharmacist consultation, motivational interviewing/coaching, drug regimen optimization, medicationreconciliation,medicationlistsorreminders,medicationsynchronization,coordinatehomenursevisits,coordinati on of care among all systems and all those involved in the care team.(25%) Provides physicians/office staff/patients and their families with accurate, adequate, and timely disease state and medication information. Clarifies any medication questions with the patient’s care team as appropriate. Collaborate with physicians, nurses, and other healthcare professionals regarding medication information and appropriate medication therapy and monitoring.OTHER DUTIES AND RESPONSIBILITIES:

Completion of a Pharm. D. degree and current licensure in the State of Michigan required (active professional and controlled substance licenses).
Specialties: Knowledge of complex/chronic disease states and therapies used within specialty pharmacy: Oncology, Multiple Sclerosis, Rheumatoid Arthritis, GI disorders, and many various inflammatory conditions preferred.
Completion of a Pharmacy Practice Residency accredited by the American Society of Health System Pharmacists (ASHP) and/or Certification from the Board of Pharmacy Specialties in a relevant area (Pharmacotherapy, Oncology), preferred.
Completion of any MyMichigan Health onsite training.
Training and competency assessment of the pharmacist occurs at a minimum yearly or more frequently, as determined by Specialty Manager
Quality assurance and review of services is completed at least yearly by pharmacist and physicians. Any concerns shall be immediately escalated to the Specialty Manager and Pharmacy Director.
Review the Collaborative Practice Agreement at least annually by the physicians, in conjunction with the pharmacist, and document review.
Attention to detail and strong organization skills, high-integrity, dependable, quality focus, empathetic. Analytical and clinical skills needed to interpret clinical data and research drug information. Knowledge and compliance with local/state/federal/contractual pharmacy regulations.
