- Discounted tuition and scholarships through Capella University Required education and experience:
- High School graduate or equivalent
- One (1) year of experience in the medical records field
- Three (3) years of experience in the medical records field.
- Experience processing release of information requests and subpoena requests require
- Collates and assembles discharged patient records according to department guidelines
- Prepares master patient index cards and files accordingly.
- Analyzes discharge records and checks content and documentation for completion and accuracy within 24 hours of discharge (72 on weekends and holidays).
- Codes and performs record abstracting with accuracy.
- Re-routes incomplete records to appropriate staff and ensures record completion by working with appropriate supervisor daily.
- Receives processes and logs any correspondence requesting information from the medical record department daily.
- Maintains an “In/Out” Log for checking out of medical records by the professional staff.
- Completes with accuracy of monthly statistical data.
- Monitors and updates medical records policy and procedures to maintain compliance with State, Federal and CARF guidelines.
- Monitors discharge records to ensure closure within appropriate time frames to meet standards.
- Monitors release of information forms for proper completion to ensure patient’s rights are protected.
- Monitors and updates privacy practices in policy and procedures to maintain compliance with State, Federal and CARF guidelines.
- Establish mechanisms for information authorization practices, controls, and internal audits of access to protected information.
- Document procedures for processing, storing, retrieving, and destroying all records that contain protected information.
- Attend team meetings and complete all training courses timely as required.
