
Social Worker - Dementia Specialist

- Responsible for the identification of social, psychological, emotional strengths and assessment needs of each resident within their assigned caseload, and focus on behavior prevention, tracking and adding interventions.
- Promote adjustment to the total living environment and respond to any resident and family questions or concerns
- Develop an ongoing discharge plan for each resident within their assigned caseload, which shall include potential for return to the community and/or transfer plans for those requesting it
- Contribute to the interdisciplinary care planning process, and contact community resources to organize for successful discharge to the least restrictive environment
- Utilize established best practices to educate employees, residents and families on dementia care, as well as develop and implement new material as approved by the Director of Social Services.
- Bachelor's degree in Social Work or related field is required
- Currently licensed in the State of Wisconsin as a Social Worker
- One year post degree experience is preferred, but not required
- Ability to relate to and work efficiently with persons who are ill, confused, disabled, and/or emotionally upset, as well as persons of diverse backgrounds and educational levels
- Knowledge of medical, social, and geriatric terminology and principles
- Ability to relate collaboratively and professionally with other staff and community agencies
