Summary:The Central Discharge Planner is responsible for the linkage between the multi-disciplinary patient care team and the Skilled Nursing Care facilities, Board and Care facilities and patient transportation resources for the provision of coordination of quality, cost-effective patient care.Qualifications:Minimum Education: High school diploma preferred.Minimum Experience/Skills: 1 - 2 years experience in Discharge PlanningPhysical Demands:
  • Considerable sitting, frequent standing/walking.
  • Lift/position up to 50 lbs.
  • Push/pull up to 50 lbs of force.
  • Reaching, stooping, bending, kneeling, crouching, manual dexterity and mobility, ability to move quickly.
