🏢 Department: LifeCircles PACE | Muskegon, Mi🎯 What You’ll Do in This Role:We are looking for a dedicated and compassionate Home Health Aide (HHA) to join our PACE (Program of All-Inclusive Care for the Elderly) team. In this role, you will provide vital in-home care and support that empowers older adults to remain safely in their homes while maintaining their dignity and independence.Primary Responsibilities:Home-Based Support:
- Provide personal care, homemaking, and respite care tailored to each participant’s needs and goals.
- Assist participants with activities that support their health, function, and overall well-being.
- Observe and report changes in condition or environment to the Interdisciplinary Team (IDT) to support timely care adjustments.
- Deliver care according to the individualized Life Plan developed by the IDT.
- Ensure all tasks align with the participant’s preferences, health goals, and plan of care.
- Serve as a vital member of the care team, communicating regularly with the IDT.
- Share insights and observations that inform participant care planning and updates to the Life Plan.
- Support participants in performing tasks safely to maintain independence and functional ability.
- Promote a sense of dignity, respect, and empowerment in every interaction.
- High school diploma or GED required.
- Certified Home Health Aide (HHA) or Certified Nurse Aide (CNA) per state requirements.
- Previous experience providing care to older adults preferred.
- Excellent communication and observation skills.
- Reliable transportation and valid driver’s license required.
