We are hiring a Health Insurance Enrollment Project Coordinator for one of our clients in Maricopa County, AZ.Job Description:
  • COMMUNITY HEALTH WORKER (HEALTHY START) MARKET RANGE TITLE: Health Services Aide
  • DEPARTMENT: Public Health
  • DIVISION/SECTION/UNIT: Community Health Action /Office of Family Health /Healthy Start
  • HYBRID STRUCTURE: Two days in field (travel in county), one day in office, two days at home
Position Overview
  • The Community Health Worker provides required Healthy Start services of recruitment and enrollment, support to case management, health and safety questionnaires, health information, and community referrals for assistance to at-risk families and hard to reach populations of South Phoenix.
Skills Required:
  • Knowledge of: Community health issues related to families.
  • Perinatal health issues.
  • Local health care and social service resources.
  • Skill in: Establishing rapport with community residents of diverse ethnic, cultural, and age groups.
  • Verbally communicating with others to convey information.
  • Communicating in writing in a clear and concise manner.
  • Adapting communication style to best fit the needs of the client.
  • Ability to: Operate computer equipment and use software programs to complete required paperwork, reports, email correspondence, and other administrative tasks.
  • Provide culturally appropriate health care information to clients.
  • Work effectively and efficiently with clients and employees.
  • Maintain regular and reliable attendance.

  • Works outside of the office to meet with families recruit and enroll eligible families into Healthy Start services; and assist in scheduling families with case manager.
  • Assists program and community participants by making recommendations, offering referrals, transporting clients to prenatal appointments, and encouraging them to seek necessary medical, social, or economic assistance and other additional resources they may need.
  • Works with participants and other partner agencies and providers in the community over the phone and in person to assure that health care and social service needs are being met. If needs are not being met, works collaboratively to establish plans that will allow families to receive needed services.
  • Provides follow-up by telephone, mail, and email to ensure that needed services are received.
  • Participates in outreach recruitment and education activities in the community by distributing brochures and informational materials, speaking one-on-one with residents, and giving presentations to raise awareness of services and issues regarding prenatal care, infant health, and ways to reduce infant mortality.
  • Works outside of the office to recruit and enroll eligible families into Healthy Start services and secure additional community resources for potential referrals.
  • Attends delivery of infants and serves as non-medical support (i.e. Doula-like) during labor when necessary.
  • Completes data entry of client documentation for follow-up, data reports, and monitoring of the Healthy Start project.
  • Participates in quality assurance, customer satisfaction, and project planning by submitting participant cases for periodic review, attending regular case management meetings with the team, and presenting various cases to the team for discussion.
  • Obtains feedback from participants on program services through surveys and makes recommendations to the management team on implementing new practices that can contribute to the success of the Healthy Start program.
Education Required:
  • Minimum education and/or experience: High school diploma or GED and two (2) years' experience working with pregnant women, infants, and/or toddlers in a social service program such as WIC, Parents as Teachers, Head Start or other home visiting program enrolling, conducting outreach and/or case management in a health or social service setting.
  • Post-secondary education may substitute for the minimum requirements on a year for year basis. Specialized training, certifications, and/or other special requirements: CPR and First Aid (adult and infant) will be required within the first six months of employment.
  • Must successfully complete a fingerprint and background screening.
  • Must possess, or have the ability to obtain by the time of hire, a valid Arizona driver's license and Vehicle Use Permit.
Education Preferred:
  • Preferred education and/or experience: Six months' experience in perinatal health care. Completion of one or more years of college.
  • Bilingual skills (English/Spanish).
  • Preferred training, certifications and/or other special requirements: Certification as a Public Health or Community Health Worker from an accredited college or currently enrolled in a Public Health or Community Health Worker program and eligible to receive certification within six (6) months of hire.
Additional Information:
  • Working conditions: May be required to work occasional evenings and weekends to participate in outreach events. Must be able to participate in emergency response training as scheduled and be available to respond as directed in the event of a public health emergency.
  • NOTE: All employees, within one month of hire, will be required to provide evidence of immunity or receive immunizations for vaccine-preventable illnesses for any position within the Maricopa County Department of Public Health.
  • Required immunizations include MMR (measles, mumps, rubella), Tdap (whooping cough and diphtheria), Varicella (chicken pox) and annual Influenza and some positions require a Hepatitis B vaccine and/or a Tuberculosis test.
