
Community Health Worker - Brown, Waukesha, Ozaukee, and Milwaukee Counties

- Help member set goals and develop a care plan to achieve those goals
- Proactively engage the member to manage their own health and healthcare
- Support the member to improve their wellbeing by staying out of the hospital, and attend regular visits to their primary physician
- As needed, help the member engage with mental health and substance use treatment
- Provide member education on community resources and benefits
- Knowledge and continued learning of community cultures and values
- Assess social determinants of health
- Create a positive experience and relationship with the member
- Conduct Health Risk Assessments (HRA)

- High School Diploma/GED (or higher)
- 6+ months of experience managing populations with complex medical and behavioral needs
- Intermediate level of proficiency with Microsoft Office applications such as Excel (create, edit, sort), Word (create & edit), and Outlook (calendar management)
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)
- Ability to travel locally within a designated area approximately 50% of the time
- Access to reliable transportation and valid Wisconsin driver's license with state-required insurance
- 1+ years of experience working with the Medicaid population
- Community Health Worker (CHW) Accreditation
- Experience with arranging community resources
- Experience with electronic charting
- Experience working in Managed Care
- 1+ years of experience of knowledge of the resources available, culture, and values in the community
- Experience working from home
- Experience with electronic charting
- Bilingual in Spanish or Hmong
