Your Role Includes:
Performing skilled nursing assessments, wound care, med management, and other treatments
Updating care plans and providing accurate EMR documentation
Teaching families about medication, care practices, and recovery goals
Collaborating with physicians and support teams to deliver seamless care
Requirements:
Active RN license in Minnesota
Valid Driver’s License and insured vehicle
Home health experience preferred but not required
