Independence Healthcare, a Modivcare Personal Care Service, is looking for a LPN Field Supervisor responsible for ensuring quality and safe delivery of home health Care services such as supervising Caregivers/Certified Home Health Aides in a patient’s home, evaluate the patient’s plan of care and assess their physical and Psychosocial needs.
Schedule: M-F 8:30AM-5:00PM
Visiting Client Territory: Lawrence, Havervill, Andover, Georgetown and surrounding areas

Pay: $33 - $35/Hourly

Performs on-site supervisory and teaching visits to evaluate caregivers
Provides clinical orientation to caregivers on assigned home care cases.
Participates in the training of new nurses and in-services for field staff.
Performs initial nursing assessments
Counsels patient/caregiver in meeting nursing and related needs.
Assigns and coordinates services provided.
Assists physician in developing a plan of treatment/care and makes necessary revisions based upon patient’s condition/needs.
Prepares clinical progress notes ensuring any changes in the patient’s condition is reported
Monitors clinical records for complete and accurate documentation.
