Designated Coordinator – Full TimeBrightPath LLC is seeking a compassionate and organized Designated Coordinator to support individuals with disabilities and older adults in community-based residential programs. This role plays a key part in ensuring services are delivered with dignity, respect, and full compliance with Minnesota’s 245D licensing standards.About the RoleAs a Designated Coordinator, you’ll oversee day-to-day service delivery for a caseload of individuals receiving supports. You’ll provide guidance to frontline staff, ensure compliance with support plans and licensing standards, and act as a liaison between program teams, families, and external professionals. The ideal candidate is organized, supportive, and passionate about person-centered care.To qualify for this role within our Residential Services programs, candidates must have prior experience as a Designated Coordinator in a setting that supports individuals with high behavioral support needs.Key ResponsibilitiesProvide oversight and coordination of services based on individual support plansTrain, support, and guide direct support staffEnsure compliance with all 245D regulations, documentation standards, and internal policiesMonitor health, safety, and progress of individuals servedLead team meetings and support goal planning for individualsCollaborate with families, case managers, and healthcare providersMaintain accurate documentation, incident reports, and progress summariesParticipate in on-call rotation and provide occasional direct care as neededQualificationsTo qualify under Minnesota 245D regulations, applicants must meet one of the following education + experience pathways:Bachelor’s degree in a human services field + 1 year of full-time direct care experience, ORAssociate’s degree in a human services field + 2 years of full-time direct care experience, ORDiploma in a human services field + 3 years of full-time direct care experience, OR4 years of full-time direct care experience under qualified supervision
