• Resumes submitted over the maxed budgeted bill rate will be rejected
• One LPN days off are Friday and Saturday, 6am-2pm, so they will work Sunday thru Thursday
• One LPN days off Sunday/Monday offer, so they will work Tuesday-Saturday

• Shift/hours: 06:00-14:00
• Submissions must include: Resume, Cover Letter, RTR, BLS/CPR certification, ODRC packet: Ohio License, US Passport or Naturalization card if born outside US
• If Missing any documents, the bid will be rejected
• If Chosen for position, TB test and HireRight drug screen must be uploaded in Dotstaff within 48 hours of start
• The posting will be closed when filled or until manager requests
• No cell phones permitted until proper paperwork has been submitted and approved
• Must clear a metal detector, no cans, tobacco, vapes, etc permitted
• Food/drink must be in clear see through containers
• No more than 1 gallon of water permitted to be brought into the facility
