Violating an out of service order while operating a CMV within the past 3 years requires a written review by safety.
Convictions or pending charges for reckless/careless driving within the past 5 years will require a written review by safety
Convictions or pending charges for leaving the scene of an accident within the past 7 years requires a written review by safety.
No DUI/DWI’s in the last 5 years
Automatic DQ’s: recent at fault rollover in the past year & No failed D.O.T alcohol/drug test lifetime
FELONIES & MISDEMEANOR:
Currently on probation or parole do not qualify for felonies
No Felonies in the past 7 years
