NURSE MANAGERPlans, organizes, and directs all nursing activities in assigned facilities. Responsible for the quality of nursing care delivered in 3 community based programs. Provides leadership to LPNs and medical support staff. Coordinates response to disaster and emergencies. Contributes to the development of policies, sets standards and plans objectives for the DD program relating to nursing in accordance with established policies. Develops staffing patterns for homes where LPN services are needed. Evaluates nursing standards and nurse protocols for DD programs. Interprets policies and communicates changes in regulations and procedures. Makes long-range plans and regularly reviews activities, problems and functions of services being delivered. Organizes and plans a systematic and continuous quality/ performance improvement program for the practice of nursing. Plans and directs the work of nursing or other professional staff and assures an adequate level of nursing care. Provides staff development including orientation, in-service training, workshops and continuing education for all levels of DD staff. Ensure that persons served receive the best possible healthcare by providing overall monitoring of health indicators as well as identifying and advocating for healthcare needs and services. Provide consultation and direction to the support services team regarding the health concerns of people served and advise agency management on the development of internal protocols that support the best possible healthcare outcomes. Develop and implement formal training programs that provide support service staff with the knowledge & skills required to protect the health and well-being of Clients and that comply with licensing standards & best practice. DUTIES AND RESPONSIBILITIES• Oversee all aspects of the healthcare needs of service recipients in the Developmental Disability Services. • Supervises LPNs and Med Tech and completes monthly LPN schedule for applicable residential home. Completes all required training with new LPNs and completes annual evaluations. Ensures that LPNs and Med Tech meet annual training and licensure renewal requirements. • Reviews medical appointment documentation, ensures that all recommendations and orders are communicated to the appropriate persons, and oversees that they are carried out. • Ensure that individuals obtain an annual physical exam, periodic dental exams, as well as other routine and follow-up medical care such as routine lab-work by providing supervision and direction to the Medical Tech who schedules and takes individuals on appointments. • Coordinate effort to ensure that all medical professional who provide services to the person served have all pertinent medical information regarding the person such as current lab-work • Coordinates the annual PPD testing and annual flu shots in all DD programs. • Coordinate discharge planning with hospitals and other medical facilities for person in the DD residential program. • As applicable, provides DD staff with feedback regarding medical appointments and doctor’s orders. • Reviews Weight logs monthly for pertinent changes in weight for DD Residential individuals and provides direction for follow-up to applicable DDPs and team. Send out monthly weights to homes. • Directs the effort for HRST completion by LPNs, Med Tech and DDPs for all DD individuals by using HRST logs, Medical visit reports and Incident reports. Does RN review of all HRSTs with Healthcare levels of 3 or above. • Attends ISP meetings for all persons who receive residential services who have precarious medical statuses. • Coordinates effort by LPNs to update MARS with all medication additions or changes as ordered and ensure that these changes are communicated to home (and day program as applicable) and then reflected on HRST. • Coordinates nutritional consults as ordered by PCP. Conduct follow-up on nutritional assessments to ensure compliance with doctor’s orders. • Does weekly visits to homes with Exceptional rate medical needs and completes required weekly and monthly documentation. • Provides training to CNAs or comparable staff working in PHC settings as needed. • Provide training to new hires and current staff as needed and scheduled in the areas of Self-Administration of Medication for the 2 applicable homes. Works with agency contracted RN to coordinate training of proxy staff in the areas of Medication Administration, Glucometer reading, G-tube flushing, and other agency approved Health Maintenance Activities. Coordinates effort for the Proxy Plan of Care renewals to be signed by agency contracted RN doing the training and by the client’s PCP. • Develop policies and protocols for medical supports and training in conjunction with DD management as needed. • Provide 24/7 medical support through their own actions or the actions of agency LPNs or proxy caregivers • Provide necessary medical direct case as needed
