Full Time Visiting Registered NurseSalaried Position based on weekly points that fit your needs. We weigh Start of Care higher, include non-patient care points for care coordination, and include additional points for high tech visits. Carelink Home Health is looking for Nurses with our standards.Coverage Areas: Kane, DuPage, Western Cook CountiesJob DescriptionRegistered Nurse - Overview:
  • Assessing and re-evaluating the patient’s condition and needs as required
  • Provide patient care according to patient’s care plan and needs
  • Teach the patient and/or family members about care and support skills
  • Communicate regularly with Clinical Care Coordinators, Interdisciplinary Care Team, and QA team
  • Accurately and thoroughly document patient care observations, interventions, and evaluations on the day services are rendered
  • Participate in and share data for Performance Improvement activities to identify opportunities for patient outcome improvement
  • Supervision of License Practical Nurse when applicable
  • Develop care plan and supervision of home health aid when applicable
  • Be an advocate for your patients and their families as you provide the tools and training to allow them to maintain a safe quality of life
Qualifications:
  • Licensed as a Registered Nurse in the state of Illinois with good standings
  • Valid Driver's License and reliable vehicle with current insurance
  • Demonstrated ability to work autonomously
  • Prior Nursing experience in a Home Health, Hospice, Rehab, or Hospital setting is required
  • Experience with Electronic Medical Records (EMR) software is preferred
  • Comfortable with technology and must have a smart phone or mobile device
  • Current CPR certification
  • Strong communication and interpersonal skills
  • Time management to balance patient care, travel, and documentation
  • Great customer service
