and help us create pathways of support—one client, one plan, one community at a time. Responsibilities:Specialist Role:
  • Act as intermediaries to hospital staff and other community mental health programs on behalf of customers and their families.
  • Collects data about patient care problems and reports it to the appropriate departments
  • Provide clinical and non-clinical screening/assessments of all customers (mental illness and chemical use/dependence) to establish the customer’s eligibility for care.
  • Assume primary responsibility for recommending specific interventions. Collaborate with local hospitals, treatment providers, courts and group homes to meet the needs of the customer.
  • Act as a liaison and advocate between customer and the community by building relationships with community partners. Advocate on customer’s behalf for the resources and access to services.
  • Establish and maintain regular schedule for visiting local hospitals and other referral sources in order to establish working relationships and to facilitate timely engagement in services for all customers.
  • Maintain clinically appropriate and timely documentation of all services provided.
Generalist Role:
  • Participates in the ongoing assessment and therapeutic engagement of all customers referred.
  • Refers clients for an in-agency comprehensive assessment to be completed within 3 days of referral and relays appropriate information, including treatment recommendations, to all treatment providers involved in the care of customer.
  • Assesses daily living skills; this includes identification of medically necessary services such as obtaining medical care, grocery shopping, maintaining clean housing, utilizing public transportation, maintaining self-care, exploring employment opportunities, etc.. Refer customers to resources that can meet their needs.
  • Transports and/or accompanies clients to appointments which connect them to community resources and services.
  • Acts as an advocate for clients to secure needed services and financial entitlements.
  • Locates and/or establishes self-help and support groups for team clients, families and/or significant others.
  • Maintains compliance with all applicable state, county and agency practice standards and guidelines.
  • Maintains client confidentiality and adherence to HIPPA requirements at all times.
  • Complete all required documentation in a timely manner consistent with CBH guidelines.
  • Attend monthly meeting in the community as identified by supervisor and Butler County Mental Health and Addiction Recovery Services Board.
  • Represent the needs, advocate for reduction of stigma and ensure that treatment goals are being addressed by attending community meetings, group home and hospital treatment team meetings as needs are identified.
  • Complete any reporting requirements from CBH and/or BCMHARS Board

  • All other duties as assigned and appropriate for the position.
Qualifications:Education: Bachelor Degree required. Preferably in human services field.Licenses or other required certifications: Minimum LPC, LSW, MFT, LPCC, LISW IMFT. Must be comfortable spending time in local hospitals and collaborating with hospital teams. Ability to build and foster external relationships is essential.Experience: Preferably 2 years of experience working in setting serving mental health clients.Specialized knowledge, skills, or abilities: Demonstrated knowledge about mental illness and addiction including engagement strategies, motivational strategies and knowledge and skill in the use of the Diagnostic and Statistical Manual of Mental Disorders recommended, but not required. Must have strong commitment to client rights.
