
Client Care Manager

- Conduct Service Inquiries and Care Consultations as needed following the consultative sales process.
- Maintain ongoing communication and a consistent follow-up schedule with potential clients and families who have yet to commit to service.
- Work with other team members to coordinate various aspects of a client’s care.
- Conduct client/CAREGiver introductions with every new client and with every new CAREGiver.
- Create and maintain client and responsible party records documenting all quality assurance meetings.
- Plan and execute a schedule that ensures each client has at minimum one quality assurance visit per (month/quarter).
- Recognize and pursue opportunities to modify service plans to best support the ongoing needs of clients through a quality assurance program.
- Achieve the annual operational plan’s revenues, gross profit, profitability and other operational goals by the stated milestones.
- Provide leadership and management of the client services team and department which affect profitability and achievement of the operational goals.
- Manage outcomes of department activities and direct changes that will drive achievement of operational goals.
- Develop and maintain a structured, measurable and inclusive Quality Assurance program that builds trust with each client and caregiver.
- Develop and maintain positive and professional relationships with referral sources in the community.
- Identify where additional support is needed and take a “hands on” approach to providing the support, whether it’s scheduling, recruiting, marketing, client care or compliance.
- When an employee is out, fill in for the employee.
- In addition to managing employees, actively participate in performing job functions needed to support the business goals.
- Travel to facilities to conduct marketing presentations and travel to client homes to conduct care consults and client care evaluations.
- Adhere to all company policies, procedures and business ethics codes and ensure that they are implemented and communicated to all employees.
- Perform On Call duties on a rotation basis or as assigned.
- Maintain and establish strong relationships with neighboring franchise owners.
- Maintain and establish strong relationships with the Business Performance team.
- Perform any and all other functions and responsibilities deemed necessary for the successful operation of a franchise office.
- College degree preferred or equivalent work experience
- Must have worked for a home care agency - home care industry experience is a must
- Four to ten years of related business experience or an equivalent combination of education and work experience may be considered
- Must possess a valid driver’s license
- This position will be responsible for supervising all of the functions performed by the Client Care Team. Responsibilities include interviewing, hiring and training employees, planning, assigning and directing work; appraising performance, coaching employee’s performance; problem resolution while seeking positive outcomes.
