- Deliver high-quality physical therapy services via Upstream's telehealth platform (Raintree Connect), ensuring accessible and effective care for patients.
- Perform virtual evaluations and re-evaluations to create and adjust personalized treatment plans with clear, measurable goals.
- Collaborate closely with patients, caregivers, and interdisciplinary healthcare teams to support communication, monitor compliance, and optimize outcomes.
- Instruct and guide patients through therapeutic exercises and functional activities to enhance mobility, strength, and overall function.
- Maintain accurate, timely, and compliant documentation within the electronic medical record system.
- Provide live or virtual mentorship and educational sessions to support the development of orthopedic skills and clinical excellence among less experienced clinicians.
- Graduate of an accredited Physical Therapy program.
- Current or pending state licensure (TX) in Physical Therapy (multi-state or compact licensure preferred).
- Minimum of 2 years of clinical experience as a Physical Therapist in an outpatient setting; prior telehealth experience is a plus.
- Strong commitment to clinical excellence, with the ability to effectively communicate and mentor within a virtual care environment.
- Reliable high-speed internet connection and a dedicated, professional workspace suitable for virtual patient sessions.
