It’s our passion.Primary PurposeThe Patient Safety/Risk Manager facilitates and conducts the activities associated with the patient safety and clinical risk management program. These key functions include but are not limited to health care risk management and patient safety foundational elements such as safety post review and investigation, investigation in depth, and investigation of case that may require root cause or apparent cause analyses as well as facilitation of root cause and apparent cause analyses and FMEAs. Facilitates projects identified through safety posts, common causes, industry trends or other means and acts as an active participant in department meetings and develops strong relationships with key leaders (nursing, physician and operational) throughout the health system.Minimum SpecificationsEducation
  • Bachelor’s Degree in Nursing, or a clinically related field of study from an accredited college or university.
Preferred Education:
  • Master’s Degree in Nursing, Risk Management, or a related field of study from an accredited college or university.
Experience
  • 2 plus years of risk management and/or patient safety experience in an acute care facility.
  • 2 plus years of clinical experience in an acute care setting.
Certification/Registration/Licensure
  • Current licensure is required in applicable field of study. Licensure must be maintained throughout employment.
  • Prefer certification in Patient Safety (CPPS) or Healthcare Risk Management (CPHRM) or test to obtain certification within 36 months of placement in position.
  • Must have Federal Emergency Management Administration (FEMA) course certification for IS-700.A: National Incident Management System (NIMS) An Introduction or obtain within 30 days of placement in role.
  • Must have Federal Emergency Management Administration (FEMA) course certification for IS-100.HCB: Introduction to the Incident Command System (ICS 100) for Healthcare/Hospitals or obtain within 30 days of placement in role.
  • Must have Federal Emergency Management Administration (FEMA) course certification for IS-200.HCA: Applying ICS to Healthcare Organizations (ICS 200) or obtain within 30 days of placement in role.
  • RNs Only: Must have current RN license or valid temporary permit with the Texas Board of Nursing; or valid Compact RN license.

  • Must be able to establish effective, respectful, and collegial working relationships with all internal and external customers including but not limited to patients, physicians, employees, students, volunteers, and vendors.

Responsibilities * Receives, manages, and investigates all assigned safety posts in Safety Center. Assesses for accuracy, severity, and trends. Works with local leaders of assigned areas as needed. Escalates cases of concern to departmental leadership in timely manner.
  • Develops strong, collegial relationships with stakeholders in assigned areas. Provides data and educational information at routine intervals. Rounds weekly on assigned areas to field questions from front line staff and validate transmission of shared learnings and previous information downloads.
  • Responsible for coordinating and conducting activities associated with all levels of investigation. Independently performs RCAs and ACAs and develops an understanding of proactive risk assessment tools such as the FMEA and all other forms of investigation.
  • Identifies deviations in clinical standards of care and patient rights violations which may have patient safety implications and/or risk exposure to the hospital. Identifies and escalates significant events through appropriate channels across the health system.
  • All investigations and reviews regardless of trigger will be entered in the event tacking log so that department productivity and metrics can be analyzed and trended.
  • Conducts Sentinel event, NQF, TDSHS and CMS investigations in accordance with departmental SOP’s.
  • Contributes and participates in patient safety initiatives for assigned areas and within the division which may minimize patient harm and/or improve patient outcomes. Assists and contributes to patient safety program development along with peer group, divisional colleagues, and immediate Patient Safety Director. Collaborates with others as needed to ensure compliance with project deadlines and/or regulatory requirements.
  • Works to maintain or exceed current job expectations and works to meet established department metrics. Can provide explanations for variances and identify actions to correct issues, as applicable.
  • Communicates professionally and effectively with all levels throughout the health system to ensure optimal uptake of assignments and sharing of information.
  • Provides education and/or subject matter expertise within the organization as needed or requested. Attends various meetings, committees, in-services and/or orientations as needed or requested.
  • Participates in required on-call schedule, including weekend and holiday Huddles to meet the needs of the department and the organization.
  • Identifies ways to improve work processes and improve external and internal customer satisfaction. Makes recommendations to supervisor, implements, and monitors results as appropriate in support of the overall goals of the department and Parkland.
  • Stays abreast of the latest developments, advancements, and trends in the field of patient safety and healthcare risk management by attending seminars/workshops, reading professional journals, actively participating in professional organizations, and/or maintaining certification or licensure. Integrates knowledge gained into current work practices.
  • Maintains knowledge of applicable rules, regulations, policies, laws, and guidelines that impact the clinical quality management department/division. Develops effective internal controls designed to promote adherence with applicable laws, accreditation agency requirements, and federal, state, and private health plans. Seeks advice and guidance as needed to ensure proper understanding.
  • Maintains certification and licensure as required.
  • Works effectively as an individual and within an interdisciplinary team structure.
  • Completes other special projects as assigned.
