
RN Care Manager

- Assess, plan, and implement medical and support services for Medicaid members post-discharge.
- Promote high-quality, cost-effective care through medication reconciliation and collaboration with care teams.
- Ensure appropriate follow-up support for members after acute care visits.
- Conduct outreach during evenings and weekends to improve continuity of care.
- Support team culture focused on achieving daily goals and connecting members to essential services.
- Strong communication skills with the ability to engage empathetically and clearly.
- Knowledge of behavioral health resources and how to effectively connect members.
- Ability to manage multiple technology systems for documentation and outreach.
- Master’s Degree in Behavioral Health or Registered Nurse with extensive psychiatric experience.
- Licensure: LMFT, LMHC, LPC, or RN with extensive psychiatric experience.
- Must hold a valid Nebraska license.
- Minimum 2 years in a behavioral health or care management role.
