Our 254-bed full-service facility offers emergency care, a primary stroke center, orthopedics and the only licensed obstetric and pediatric units in Charlotte County.The role youll contribute:The Care Management Supervisor is under the general leadership of the Care Management Manager or Facility Director and assists in monitoring and performing the daily operations and supervision of team members in the Care Management department. The Care Management Supervisor is responsible for the daily front line operations as assigned by the Care Management Manager or Facility Director. The Care Management Supervisor will serve as the primary point of contact for Care Managers, Social Workers and patients, to provide mitigation for any barriers/concerns that may arise on the inpatient units. The Care Management Supervisor will be responsible for supporting positive and safe patient outcomes, employee evaluations, and customer experience.The value youll bring to the team:Supervises daily operations of discharge planning and social services in the inpatient and emergency department setting as directed by Care Management Manager or Facility Director, including:
Direct supervision of Care Management staff
Primary point of contact for inpatient unit Care Management related barriers/concerns
Manages own assignment and patient/department outcomes of those under direct supervision
Assists Care Management Manager/Facility Director to implement the Corporate Care Management strategies QualificationsThe expertise and experiences youll need to succeed:Associate's of Nursing
