Coordinate the scheduling and tracking of ordered referrals for specialty, diagnostic and therapeutic services
Work closely with medical staff in procuring referral appointments for patients
Sending and receiving necessary information to/from outside venders and clinics
Prepare patient correspondence
Utilize scheduling and patient database software to provide services
Ability to track and run reports for Quality measures
Initiate communication with patients that need preventive care, missed screenings, immunizations, etc. (Gaps in Care)
Evaluate effectiveness of healthcare needs and follow-up within the referral process or and Quality measure collection
Assist with scheduling patients as needed and for Annual Wellness Visits
Participate in performance improvement activities
Other duties deemed necessary by supervisor
KNOWLEDGE, SKILLS, AND ABILITIES
Excellent typing, computer, transcription and organizational skills
Knowledge of medical terminology
Knowledge of Quality measures
Knowledge and experience with personal computer software technology
Strong communication skills
Ability to maintain strict confidentiality
Ability to prioritize own work and work independently
Knowledge and respect of the American Indian culture and other culturally diverse populations we serve
EDUCATION AND EXPERIENCE
High School diploma or equivalent
Experience in a healthcare setting
