You'll work closely with medical providers to ensure holistic care.️ ResponsibilitiesCoordinate care with medical providers and mental health professionalsScreen and assess patients for mental health and substance abuse disordersProvide education and support for treatment optionsTrack treatment progress and monitor symptomsSupport medication management and adherenceDeliver brief behavioral interventions using evidence-based techniquesFacilitate referrals and consultationsDocument patient progress and treatment plansRequirementsMasters-level licensure in a behavioral health field (e.g., LCMHC, LCSW, LMFT)Masters-level associate licensure candidates (e.g., LCMHC-A, LCSW-A, LMFT-A)Experience in mental health screening and treatment planningStrong communication and teamwork skillsComfortable working with patients in person and by phoneFamiliarity with evidence-based interventions and psychopharmacologyDetail-oriented with solid documentation habitsCompassionate and approachable demeanorWhy Join UsBe part of a growing practice making a real differenceWork with a diverse population in a supportive environmentHelp establish a pioneering CoCM program in an underserved areaBenefitsCompetitive salaryNo weekends or eveningsopen to hybrid with 3 days in officereimbursement for CEUs️ Equal Opportunity Employer
