Immediate need for a talented Senior LTSS Service Care Manager RN. This is a 06+months contract Possible 2 Hire opportunity with long-term potential and is located in San Antonio TX (Onsite).   • Completing SAI assessments at member home.
  • Telephonic outreaches
  • Care Coordination
  • Following making/follow up on requests for DME, Therapies, provider services etc.
  • 28 day follow ups
  • Documentation and forms
  • Describe the performance expectations/metrics for this individual and their team:
  • Follow work processes
  • Complete assessments, outreaches and follow ups with in turn around times.
  • Tell me about what their first day looks like:
  • First day will be getting set up with equipment and doing trining
Key Requirements and Technology Experience:
  • Key Skills; Pediatrics, Home Health
  • RN with experience in pediatrics
  • Knowledgeable of word ,excel, outlook and teams
  • Communication skills
  • Education/Certification BSN or ADN
  • Requires Graduate from an Accredited School of Nursing or Bachelor's degree in Nursing and 2 - 4 years of related experience
