- Strong knowledge or prior experience in payor communication/requirements and troubleshooting processing
- Medical Terminology
- 3-4 years experience Clerical office or hospital clerical or care delivery
- Up to 1 year experience in Healthcare either in a hospital or community agency
- Advanced Computer skills including Data entry; Microsoft Office(Word, Excel)

- Works under direction of Care Management (CM) RN and Social Worker (SW) who delegates tasks and continues to monitor progression and completion.
- Assists with making referrals as directed by the RN CM and SW for post- acute care including but not limited to Home Health, Durable Medical Equipment, Skilled Nursing, Assisted Living, etc.
- Alerts the appropriate staff members when patients have been accepted to post-acute level of care or services
- Assists the RN CM or SW in communicating with insurance companies regarding barriers to authorization of post- acute needs.
- Assists the RN CM or SW to determine financial/payer status working with Registration and Financial Counselor/Assistants.
- Utilize effective communication skills and engagement with interpersonal skills in interactions with patient’s family and healthcare team.
- Performs clerical functions related to discharge planning process such as faxing, copying, discharge packets, etc.
- Schedule follow up care or appointments as directed by the RN CM.
- Delivers the Important Message from Medicare (IMMs) as directed by RN CM and SW
- Supports RN CM and SW by assisting with completion of forms as needed
- Support Quality Initiatives under the direction of department leadership
- Demonstrate customer focused interpersonal skills, utilizing problem solving process.
- Communicate, resolves or when appropriate escalates to social worker barriers with work flow and processes.
- Performs other duties as assigned; for example, participation in planning sessions for departmental activities.
- Assists manager and SW supervisor as needed with clerical and office management including organizing data collection and record keeping.
- Navigates several software programs to assist team.
- Communicates using email; teams chat; text being available timely.
- Coordinates or arranges transportation for the patient’s discharge to include medical transport, Circulation Lyft, RTA bus passes, taxi, etc.
- Obtains necessary insurance authorization for transportation
- Maintain licensure by obtaining CEUS if appropriate.
- Attend appropriate workshops, inservices and departmental meetings
- Associate degree. Equivalent experience accepted in lieu of degree.
