We are currently seeking a dedicated and experienced School Transition and Placement Coordinator to join our Chicago campus team.About the Role
The School Transition and Placement Coordinator plays a vital role in supporting students as they move from HPDS to schools in their home communities. This position involves collaborating with families, educators, and service providers to ensure each student's transition is as seamless and successful as possible. You will assess individual student needs, identify appropriate school placements, coordinate necessary supports, and provide follow-up services after students have transitioned. In addition, you will plan and manage graduation activities, from ceremonies to yearbook production, helping our students and families celebrate their achievements.Key Responsibilities
While every day will bring new opportunities to make a difference, your work will include:Coordinating educational placements for students transitioning from HPDSMeeting with parents, teachers, and service providers to ensure alignment with each student's learning needsLeading and supporting the IEP process for transitioning studentsOrganizing and overseeing graduation events and related activitiesMaintaining ongoing professional development and sharing best practices with colleaguesRepresenting HPDS at meetings with partner schools and in the broader educational communityQualifications
The ideal candidate will bring:A bachelor's degree or higher in learning disabilities, special education, or social work (Master's preferred)Proven experience placing students with learning disabilities in appropriate educational settingsKnowledge of public and private schools within the Chicago metropolitan areaAt least three years' experience working with children with diagnosed learning disabilities (preferred)Strong organizational, problem-solving, and communication skillsThe ability to work effectively and efficiently while managing multiple prioritiesA warm, approachable demeanor that fosters trust with students, parents, and colleaguesWhy Work With Us
