Daily Responsibilities:
Deliver comprehensive nursing visits and care planning
Monitor patient outcomes and adjust interventions as needed
Coordinate services with physicians, therapists, and care aides
Lead family education and health management guidance
Requirements:
RN license in MN
3 years RN experience (home health preferred)
Reliable car and driver’s license
