Your focus on safety, empathy and clear communication ensures every visit supports patient independence and comfort.Key ResponsibilitiesPatient Assessments & CarePerform vital signs, wound care and medication administration per care planEducate patients and families on self-care strategies and medication useCoordination & DocumentationWork closely with RNs, physicians and therapists to coordinate seamless careComplete accurate, timely EMR documentation after each visitSafety & ComplianceFollow infection-control protocols, HIPAA rules and state/federal regulationsHandle medical supplies and equipment safelyTeam Collaboration & GrowthParticipate in case conferences, team huddles and peer mentoringPursue continuing education to stay current with best practicesA Typical DayReview patient assignments and gather suppliesTravel to home visits, deliver nursing care and build rapportDocument findings and update care plans in real timeCheck in with your supervisor or RN for feedback and hand-offsUse any downtime for training, chart maintenance or case reviewsQualificationsValid Ohio LPN license and graduate of an accredited LPN programMinimum 1 year of home health or equivalent clinical experienceProficient in wound care, medication administration and patient teachingFamiliar with EMR systems (e.g., Kinnser, McKesson) and mobile chartingStrong communication, time-management and problem-solving skillsComfortable adapting to varied home environments and schedulesWork EnvironmentFast-paced, patient-centered field role with office check-ins as neededFlexible schedule including occasional evenings or weekendsCompany-issued mobile device for real-time charting and communicationSuccess MetricsHigh patient and family satisfaction scoresAccurate, on-time documentation and care-plan complianceCompletion of all scheduled visits within productivity targetsActive participation in team learning and process improvementInclusion & Belonging: VNA of Ohio is an equal-opportunity employer.
