In this work environment, all team members are part of a community and appreciated by the people they work with – residents and coworkers – every day.Summary:A Client Advocate is responsible for supporting and working collaboratively with care managers by assisting with client calls, making referrals, providing supportive client visits, and ensuring monthly billing charges are submitted; The goal of this position is to build a trusted relationship with each client and assist with coordination of services to improve the clients’ home, community, and well-being so they may stay in their home for as long as they choose.Essential Duties and Responsibilities:1. Builds trusted relationships with each client as assigned by an EverTrue care manager (“Social Worker, MSW”); This will be accomplished through regular visits, phone calls and other communication with each client2. Listens attentively to the client and their needs, consulting with care manager on how to support client3. Always demonstrates respect for the client and upholds HIPAA requirements4. Works with care manager and client on helping clients meet their identified personalized goals5. Documents each interaction in current documentation system and updates care manager on any client change in condition, situation, or goal6. Supports clients by understanding the care plan goals7. Monitors and tracks the number of times they and/or their care manager has contact with each client each week and ensures documentation is up to date and that services are billed accurately8. Maintains an understanding of the model of service provided; Supports the marketing of the Anywhere Care care management program9. Assists their care manager coordinate services on behalf of clients that are appropriate; Ensures clients understand cost of services10. Ensures that clients are provided choice when hiring outside services by reviewing options with each client for them to choose from11. Have corrected vision and hearing within normal range and have manual dexterity with normal range of motion of all extremities. May be requested to work weekends, holidays, and occasional overtime. Must be able to work alone without usual support systems immediately available in acute care settings.
