- A qualified professional, who directs, supervises, evaluates and provides physical therapy services to adult patients in the home, following physician’s orders.
- Evaluate patient using diagnostic muscle, nerve, joint, and functional ability tests and measures.
- Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
- Recommend modification of plan of care to physician, as needed.
- Document clinical notes on the date of the visit into medical record and keep all records and summaries up-to-date.
- Treat patients to address impairments and functional limitations, with a goal of reaching maximum functional independence.
- Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic, and durable medical equipment (canes, walkers, wheelchairs, etc.).
- Include and educate the patient and caregiver(s) regarding treatment plan and goals.
- Attend physical therapy staff meetings and in-service presentations.
- Participate in case conferences.
- Participate in the development and periodic revision of the physician’s Plan of Treatment.
- Assure appropriate safety practices and maintenance of equipment.
- Other duties as assigned.
- Graduate of an accredited Physical Therapy program.
- Current, valid Physical Therapy license issued by NYS Department of Education.
- One year of practical experience as a licensed physical therapist.
- The Physical Therapist is primarily in a non-office setting and may be exposed to outdoor conditions.
- Medium work - Exerting up to 50 pounds of force occasionally, and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body. Medium work involves sitting and standing.
