- Responsible for utilization review, discharge planning and care coordination with the patient, patient’s family, physician and health care team.
- Works with other members of Continuum of Care Department sharing responsibilities as assigned to meet the needs of each patient.
- You will plan, implement and evaluate patient care plans before, during and after the procedure to optimize outcomes and maximize available resources.
- Graduate of an accredited school of nursing required.
- Current state nursing license required; and/or has an active compact license, in accordance with law and regulation
- (RN) Registered Nurse
- Registered Nurse Diploma
- Minimum of one year of clinical nursing experience preferred.
- Two years Managed Care and/or Hospital Case Management experience preferred
