This position partners closely with physicians, nursing staff, and various departments to ensure patients move efficiently along the continuum of care, promoting optimal clinical outcomes and high levels of customer satisfaction.Responsibilities include entering and maintaining accurate patient information in the electronic health record system, facilitating timely communication between key stakeholders, and supporting smooth and efficient patient transfers within the hospital. Qualifications:Requirements:High school diploma or equivalent required6 months to 1 year of experience in patient-related clerical or customer service rolesExperience with medical terminology, medical office procedures, or patient registration requiredStrong interpersonal and communication skillsProven ability to work independently and manage tasks with minimal supervision Responsibilities:Collaborates with nursing staff and other departments to coordinate and assist with efficient patient flow and placement from all sourcesEnters patient information accurately into the electronic system (Memorial Access) and collects basic patient data for the RNMaintains and updates electronic systems with current and pertinent patient information, ensuring accurate and timely documentationUtilizes communication tools such as pagers and phones to coordinate and ensure smooth patient placement and transitionsAnswers incoming phone calls and processes messages received via fax and printerCommunicates and collaborates effectively with nursing units, physicians, departments, and outlying facilities to facilitate timely and efficient patient placementSupports patient flow process improvements and partners with the Patient Placement team to identify and implement quality improvement opportunitiesDemonstrates flexibility, organization, and the ability to multitask effectively in high-pressure situationsUnderstands and adheres to hospital, departmental, and Joint Commission policies and procedures related to patient placementDemonstrates proficiency in basic computer operationsPerforms other related duties as assigned
