The Licensed Practical Nurse (LPN) provides direct skilled nursing care to clients in their homes under the supervision of a Registered Nurse (RN). The LPN supports client well-being by implementing care plans, delivering nursing interventions, and reinforcing the RN’s leadership through staff guidance and observation.II. Responsibilities
Support client health and safety by implementing care plans developed by the RN and physician.
- Deliver skilled nursing interventions, such as wound care, catheter care, medication administration (within scope), and health teaching.
- Observe and report client responses, changes, or concerns to the supervising RN promptly to ensure appropriate care adjustments.
- Provide day-to-day supervision and direction to PCAs, CNAs, and other unlicensed personnel as delegated by the RN.
- Contribute to staff development by reinforcing training and modeling professional standards of care.
- Maintain accurate and timely documentation that supports continuity of care.
- Participate in supervisory visits with the RN to evaluate care delivery and ensure compliance with agency standards.
- Foster supportive relationships with clients and families, ensuring they feel confident and reassured about care provided.
Clients consistently receive nursing care that is safe, effective, and aligned with the RN’s plan of care.
- Early identification and reporting of client condition changes lead to timely interventions and improved outcomes.
- Staff provide reliable care under the LPN’s daily guidance, maintaining continuity and trust in the team.
- Families experience peace of mind through clear communication and professional, compassionate support.
- Documentation accurately reflects the care delivered and ensures smooth coordination with the RN.
- The agency is recognized as responsive, compliant, and client-centered due in part to the LPN’s dependable care delivery.
Current Georgia LPN license in good standing.
- Current CPR and First Aid certification.
- Demonstrated ability to deliver high-quality nursing care within scope of practice.
- Strong observational skills with the ability to recognize and report client condition changes.
- Effective team collaboration skills with a willingness to support and guide unlicensed personnel.
- Excellent interpersonal and communication skills with clients, families, and staff.
- Reliable documentation skills that contribute to continuity of care and compliance.
- Reliable transportation and willingness to travel to client homes as needed.
