2. Drug Information Resource - Provides pertinent drug information to medical and ancillary hospital staff. Researches medication dosages, indications, interactions, compatibility, and administration when requested by healthcare professionals. Responds timely and in a professional manner.
3. Departmental Clinical Program Participation - Completes pharmacy consults in a timely and accurate manner. Attends multi-disciplinary rounds.
4. Technician and Ancillary Staff Supervision - Reviews medications prepared for distribution by technicians with respect to accuracy, appropriateness, and timeliness. Monitors technician accountability to follow established policies and procedures for medication delivery, including the automated dispensing system. Reviews technician preparation of intravenous admixtures, parenteral nutrition compounding, chemotherapy and miscellaneous sterile products. Ensures and validates processes are followed for dispense prep of medications. At a minimum, departmental dispense prep must be above 84 % and one technician pearl must be identified and completed to achieve successful.
5. Departmental Performance Improvement Participation - Participates in the departmental improvement process on a routine basis by ensuring that all quality control measures are followed and properly documented. This includes but is not limited to: monitors ADC Report Reviews (ex: override and discrepancies), monthly narcotic inspections. Supports the High Reliability Organization activities (ex: Safety First Reports, Near Misses, Performance Improvement)
6. . Controlled Substance(s) Accountability - Completes all chain of custody related processes in regards to the dispensing of controlled substances. This includes but is not limited to: appropriate dispensing, storage and hand-off of medications prior to delivery, and monitoring, investigating and clearing of all open transactions involving controlled substances.
7. Adverse Drug Reporting - Reviews, identifies, reports and documents potential ADRs.
