- Evaluate and treat program participants (older adult and geriatric) on acute, restorative, or maintenance levels of care.
- Develop and integrate physical therapy treatment plans into the Plan of Care (POC) as directed by physician orders.
- Collaborate with the interdisciplinary team, caregivers, and facility staff to promote safety, mobility, and independence.
- Provide patient and caregiver education on therapy plans, DME (Durable Medical Equipment), and home exercise programs.
- Supervise and delegate therapy interventions to PTAs and Rehab Aides in alignment with state regulations.
- Conduct home visits and educational sessions as needed, documenting all care in participant records. This will be 10% of your time.
- Bachelor’s Degree in Physical Therapy (Master’s or Doctorate preferred).
- Graduate of an accredited Physical Therapy program.
- Active PT license in the state of practice.
- 1+ year of experience working with the frail or elderly population (required).
- Strong communication and collaboration skills, with experience working in interdisciplinary care teams.
- $41.73–$54.28/hr (compensation may vary depending on experience).
- Monday–Friday, 8:00 am – 4:30 pm.
- 10% local travel to participants’ homes and nearby facilities
